Elevation Hospice and Home Health is a licensed hospice provider in the State of Utah, with a certification from the Center for Medicare & Medicaid Services. We also have added several commercial insurance companies as we are expanding our growth. Elevation Hospice is committed in providing hospice services through an interdisciplinary team of professionals and volunteers.
